My Child has my permission to go to Bright Lights at Calvary Chapel Westside. I understand that there will be various activities at the church and other locations. I absolve Calvary Chapel and those representing the church from any responsibility for any accident involving my child during the Bright Lights Summer Outing. I also authorize the leadership of Bright Lights to seek necessary medical assistance in the case of emergency involving my child.
